No Collar Policy Form

For the safety of our guests at the Inn or Salon, as well as our hospitalized patients, we require collars and leads be removed and returned to the owner at the time of admission into the practice regardless of their length of stay. This applies to all collar types, including but not limited to: flea and tick collars and calming collars, various harnesses, gentle leader or haltie type collars, and all lead types. All of our guests and patients are transferred using a slip lead.

I understand and agree that all collars, harnesses and leads were returned to me prior to my pet's admittance.
